Share Registry Forms

All shareholder records are held and maintained by the Company’s share registrar, Advance Share Registry Services, of 110 Stirling Highway Nedlands WA 6008. Shareholders may contact Advanced Share Registry Services using the internet link provided, or by facsimile on +61 8 9389 7871 or by telephone, between the hours of 9am and 5pm AWST Monday to Friday, on +61 8 9389 7871.

Change of Shareholder Details

Shareholders are encouraged to keep their details current by advising the share registrar promptly of any change, and particularly changes of address for mail and email. Failure to do so may mean that information relevant to the shareholding is not received. The forms on which changes can be notified are as follows:

Shareholder Election Forms

The Australian government has passed legislation that assists shareholders and companies to protect the environment by minimizing document printing and to conserve funds by minimizing both printing and postage costs. Shareholders wishing to support these objectives are encouraged NOT TO ELECT to receive a hard copy annual report, but TO ELECT to receive other communications by email and in this instance to provide their email addresses. In this regard it is important to note that shareholders may change their election at any time, and any number of times. The forms on which elections can be notified are as follows:

Share Transfer

Shareholders selling shares through brokers will generally not be obliged to complete this form. However, it must be completed in all other instances, such as off-market transfers.
